Hearts of Iron IV sa ocitnete znova v druhej svetovej vojne, v ktorej budete môcť zmeniť beh udalostí a prepísať dejiny. V hre budete môcť ovládať ktorýkoľvek štát na svete od Luxemburska a Nepálu, až po USA, Nemecko a Sovietsky Zväz a je na vás, či sa pokúsite ísť historickou cestou alebo sa od nej odkloníte. Okrem zadávania rozkazom divíziám, z ktorých sa skladá vaša armáda, budete mať na starosti obchodovanie so surovinami, diplomaciu, výrobu armádneho vybavenia, spravodajskú službu a tak ďalej. V hre sa objavia dobové osobnosti, boje budú prebiehať na zemi, na vode aj vo vzduchu s novými zbraňami hromadného ničenia a priestor dostane aj diplomacia a obchodovanie.
